Planning for Your Visit
Knowing what to expect can help your visit go more smoothly.
What you should bring to your appointment:
- Photo ID
- Insurance Card
- List of medications you are taking
- Referral *
- Co-Pay *
- Medical records that you think may be helpful
Please contact your insurance carrier to find out if a referral is required. Please call your primary care physician as soon as you have made your appointment with us. If you do not get approval from your insurance carrier before your visit, your insurance company may not cover our services.
Depending on your insurance you may be responsible for a co-payment. The copay amount can be found on the front of your insurance card.
Please talk to your doctor about arrangements so that copies of any tests or x-rays previously done will be available at the time of our visit.
